The thoughts are not who you are
The most distressing feature of OCD is often the content of the intrusive thoughts themselves — violent, sexual or blasphemous themes are extremely common, and people frequently carry them in silence for years believing the thoughts say something terrible about them.
They do not. Intrusive thoughts in OCD are distressing precisely because they are contrary to your values. That is what distinguishes them, and clinicians who treat OCD are not shocked by them.
What treatment can involve
- Evaluation that distinguishes OCD from generalized anxiety, which it is often mistaken for
- Exposure and response prevention principles — the best-evidenced psychological treatment for OCD
- Medication where appropriate, often at doses higher than those used for depression
- Reducing reassurance-seeking, which feels helpful and maintains the cycle
- Treating co-occurring depression, which is common with OCD

OCD: common questions
- My intrusive thoughts are disturbing. Will you think I'm dangerous?
- No. Distressing intrusive thoughts are a recognised, well-documented feature of OCD, and the distress they cause is itself the clearest sign they run against your values. This is familiar clinical territory.
- Is OCD just about being tidy?
- No, and that stereotype keeps people from seeking help. OCD is a cycle of intrusive thoughts and compulsions that can consume hours a day and has nothing to do with liking things neat.
- Can OCD be treated online?
- Yes. The core psychological treatment works well over video, and medication management is straightforward by telehealth.
- How long does treatment take?
- Meaningful improvement usually takes months rather than weeks, and it is gradual. Most people with OCD improve substantially with proper treatment.
