Terms and Conditions of Use

The terms that govern this website and the care we provide through it — written to be read, not to be skipped.

Effective September 18, 2026

These Terms and Conditions of Use (“Terms”) govern your use of holisticmedunited.com and the telehealth services offered through it. They should be read together with our Consent to Telehealth Services, our Billing, Refund & Cancellation Policy and our privacy policy, each of which forms part of these Terms. Where a conflict arises, the more specific document governs the subject it covers.

1. Who we are

This website is operated by Holistic Medical Services United, LLC, a self-pay telehealth practice. Clinical care is provided by Shumeka Hill, DNP, PMHNP-BC, FNP-BC, CPNP-AC, LCDC II, who holds active licensure in the 23 states listed on our where we practice page.

Our address at 11180 State Bridge Rd, Ste 204, Alpharetta, GA 30022 is an administrative office. It is not a clinic, we do not see patients there, and it is not a walk-in location. All care is delivered by video.

2. Accepting these terms

By using this website, creating an account, or booking an appointment, you agree to these Terms. If you do not agree, please do not use the site or our services.

3. Not medical advice

The information on this website is general health information. It is not medical advice, it is not a diagnosis, and it is not a substitute for an evaluation by a qualified provider.

Reading this site, contacting us, or submitting information through the site does not by itself create a provider–patient relationship. That relationship begins only when you are accepted as a patient and an evaluation takes place. Treatment decisions are made between you and your provider after that evaluation. Individual results vary.

4. Emergencies

This is not an emergency service. We do not monitor this website, email or the patient portal continuously, and we cannot respond to emergencies through any of them.

If you are in crisis, call or text 988 for the Suicide & Crisis Lifeline. In a medical emergency, call 911 or go to your nearest emergency department.

5. Who may use these services

  • You must be physically located in one of the 23 states where your provider is licensed at the time of each appointment. This is a legal requirement that applies to every telehealth provider, and we cannot see you outside those states.
  • You must be 18 or older to create an account. Care for a minor may be arranged by a parent or legal guardian, subject to the consent rules of the state the minor is located in, which vary.
  • You must provide accurate information about your identity, location, medical history and current medications. A valid government-issued ID is required to receive services.
  • We may decline to accept you as a patient, or may determine that telehealth is not clinically appropriate for your situation and recommend in-person care instead.

6. Accounts and the patient portal

Scheduling, records and messaging are provided through our electronic health record vendor. You are responsible for keeping your login credentials confidential and for activity that occurs under your account. Tell us promptly if you believe your account has been accessed by someone else.

7. How we communicate

  • The patient portal is the secure channel for anything clinical. Please use it for health information, questions about treatment, and documents.
  • Email is not a secure channel. Please do not send detailed health information by email. Anything you do send by email is sent at your own risk.
  • By giving us your phone number or email address, you agree that we may contact you about appointments, billing and your care. Phone hours are Monday to Friday, 9:30 AM to 5:30 PM Eastern. Message and data rates may apply, and you can opt out of non-essential messages at any time by replying STOP or by emailing Hello@holisticmedunited.com.
  • We cannot guarantee a response time to non-urgent messages. See Section 4 for emergencies.

8. Payment

We are a self-pay practice. We do not accept or bill insurance, we do not submit claims, and we do not verify coverage. You are responsible for the full cost of your care, which you confirm in our Patient Self-Pay Agreement at intake. Fees, the card on file, cancellations, no-shows, refunds and payment disputes are all governed by our Billing, Refund & Cancellation Policy.

9. Prescriptions

No prescription is promised or guaranteed. Medication is prescribed only where clinically appropriate following an evaluation, in the professional judgement of your provider, and in accordance with federal and state law. This includes controlled substances, which carry additional legal requirements and are never guaranteed in advance of an evaluation.

Booking an appointment is not a request that must be fulfilled, and paying for a visit pays for the evaluation, not for a particular outcome or medication.

10. Acceptable use

You agree not to:

  • Use the site or our services for any unlawful purpose, or to harass, threaten or impersonate anyone
  • Attempt to gain unauthorised access to any account, system or data
  • Scrape, harvest, copy or republish the contents of this site at scale, or use it to train a machine learning model, without our written permission
  • Interfere with the operation or security of the site, or introduce malicious code
  • Misrepresent your identity, your location, or your medical history in order to obtain treatment or a prescription

Our Billing, Refund & Cancellation Policy also sets out our zero-tolerance policy for abusive behaviour toward providers, staff and other patients.

11. Our content and yours

The text, design, images and other material on this site belong to Holistic Medical Services United, LLC or are used with permission. You may read, print and share it for your own personal, non-commercial use. Everything else is reserved.

Anything you submit to us — intake forms, history, messages, documents — remains yours. You grant us permission to use it for the purpose of providing your care and operating the practice, as described in our privacy policy.

12. Third-party services

We use third parties for scheduling, records, payments and communications. Those services have their own terms, and we are not responsible for how they operate. Where a third party handles protected health information on our behalf, we require a Business Associate Agreement.

Links to other websites are provided for convenience and are not an endorsement. We have no control over their content.

13. Disclaimers and liability

This section is about the website. Nothing in these Terms limits or excludes any liability for the clinical care we provide, for professional negligence, or anything else that cannot lawfully be limited. Clinical care is governed by the applicable standard of care and by state law, not by these Terms.

Subject to that, the website and its content are provided on an “as is” basis. We do not warrant that the site will be uninterrupted, error-free or secure, or that its general health information is complete or current for your situation.

To the fullest extent permitted by law, and subject always to the carve-out at the start of this section, our total liability arising out of your use of this website is limited to the greater of the fees you paid us in the twelve months before the claim or $500, and we are not liable for indirect or consequential loss. These limits do not apply to gross negligence or wilful misconduct.

14. Ending the relationship

You may stop using the site and our services at any time, and you may withdraw your consent to telehealth at any time without affecting your right to future care, as described in our Consent to Telehealth Services.

We may end a clinical relationship where it is clinically appropriate or where our policies have been breached. If we do, we will follow the notice and continuity-of-care requirements that apply in your state, which may include a period of continued availability for urgent needs and assistance identifying another provider.

15. Governing law

Talk to us first. If you have a complaint, please raise it with us at Hello@holisticmedunited.com before starting any formal proceeding, and give us 30 days to put it right. Most problems are quicker to fix directly, and this step costs you nothing.

If that does not resolve it, these Terms are governed by the laws of the State of Georgia, without regard to its conflict of law rules, and the courts located in Georgia will have jurisdiction.

We do not require you to arbitrate, and we do not ask you to give up your right to bring or join a class action. Nothing here affects any right you have under the law of the state you are located in that cannot be waived by agreement, including your right to complain to that state’s licensing board, and nothing here prevents either of us from bringing a claim in small claims court.

16. Changes to these terms

We may update these Terms. The effective date at the top of this page tells you when they last changed. Material changes will be notified to active patients through the portal or by email. Continuing to use the site or our services after a change means you accept the updated Terms.

17. General

  • If any part of these Terms is found unenforceable, the rest continues to apply.
  • If we do not enforce a provision, that is not a waiver of it.
  • You may not transfer your rights under these Terms. We may transfer ours in connection with a sale or reorganisation of the practice.
  • These Terms, together with the documents referred to in them, are the entire agreement between us about the website and our services.
